Adobe Acrobat Fillable Forms
The Alabama Fire College registration forms are Adobe Acrobat
fillable forms. You will need to download the most recent Adobe Acrobat
Reader to take full advantage of this form.
If you would like to be able to save the registration forms in digital
format to your PC you will need to purchase Adobe Acrobat software. Once you have opened the form in
Acrobat Reader mouse click in the first cell and input text required. To
move to the next cell, simply use the Tab key to advance one space.
At the top of the
registration form
 is a check box to
indicate which program you will be registering under. Your choices are
Resident Program (courses taught at the Tuscaloosa campus), Field Programs
(courses taught in local departments, these registration forms are turned
into the department hosting the course), and Industrial Programs (courses
scheduled for the Industrial client, these registration forms are turned
into the client sponsoring the course).
*Please note the second data entry line is pertinent to the
Field Programs only.
Date fields have an automated filter. Example: to key in the
date for October 10, 2002 you simple enter 10-10-2002. You
must put dashes or forward slashes between month, day, and year.
You must enter in all four digits of the year. When
you tab the date will be displayed at 10/10/02.
Social Security numbers and telephone numbers also have a
filter. Key in your social security number and telephone number without any
dashes, and once you tab, they will be displayed correctly.
When typing in your credit card information, you only have to
put month and year in the expiration date field. You
must enter all four digits for the year on expiration date.
