Documentation of New
Personnel
Recently we have had
fire fighter candidates arrive for Fire fighter I schools without the
Fire College having the necessary documentation. This causes problems
for the Alabama Fire College, the host of the school, the candidate, and
the employing agency. The candidate can be withheld from attending until
the necessary documentation is received.
Section 360-X-1-.01 of
the General Regulations and Administrative Procedures states:
- The chief shall, within ten (10)
days after the date of employment of a new employee, file with the
office of the Commission a completed AFC & PSC Form 04 “Notice of
Employment”.
- Also, the AFC & PSC Form 05
“Physical Statement” completed and signed by a physician licensed to
practice in the state of Alabama.
- Documentation of successful
completion of the CPAT (Candidate Physical Ability Test), or other
such validated physical ability evaluation prior to entering a
minimum standards course for Firefighter I.
The above documentation
must be received by the Alabama Fire College and Personnel Standards
Commission prior to the Firefighter I candidate attending any part of a
Firefighter I school.
